General Conditions

A form of identification, such as a driver's license, must be provided prior to your appointment.

In order to keep your appointment, you are required to pay the cost associated with the visit type prior to your visit.

Check-in documentation is required prior to your appointment, including general consent forms for treatment. If these forms are not completed, your appointment may be canceled or rescheduled.

In the unfortunate event you are not able to make your appointment, 24 hours noticed is required or a $25.00 fee will be applied.

Being late disrupts the provider's schedule and can cause delays for subsequent patients. Providers often have back-to-back appointments and cannot extend your session time significantly. If you arrive 15 minutes late to your telehealth session, your appointment will be canceled your deposit will be kept. To avoid issues, it's recommended to log in to your virtual appointment 10 to 15 minutes early to sort out any technical difficulties.

We reserve the right to refuse service to anyone for any reason at any time. You agree not to reproduce, duplicate, copy, sell, or exploit any portion of the Service without express written permission.
